Please bring the following to the examination
 
We will generate your I-693 form electronically
Official Photo ID, preferably your passport or a U.S. driver’s license or state ID
(Refugees: Bring travel document or I-94 card)
Payments by cash or credit card only. (All credit cards accepted)
Medical and vaccination records, if you have any.
You should have something to eat and drink before arriving for your examination, but please do not bring food into the office
A urine specimen is not needed.
