1 to 14 years old $ 100
Vaccination records required
Fee includes tuberculosis skin test (PPD) or chest x-ray if necessary for completion of I-693 medical form
Any vaccinations administered by our office are at additional cost
15 and older $ 200
Vaccination records required if 18 years old and under
If 19 or older, vaccination records are useful but not required
Fee includes blood testing for HIV and Syphilis; Tuberculosis skin test (PPD) or Chest x-ray; Vaccination for Tetanus; antibody testing for Measles, Mumps, Rubella and Varicella; and completion of I-693 form *
* Fee does not include other vaccination if required
If you are given a tuberculosis skin test (PPD) by us, you must return to our office 48 to 72 hours later to have your arm examined by us before completion of the I-693. If your skin test is reactive *
* You will then be given a chest x-ray here before completion of the I-693

 Instructions
 
Please bring the following to the examination:
 
We will generate your I-693 form electronically
Official Photo ID, preferably your passport or a U.S. driver’s license or state ID
(Refugees: Bring travel document or I-94 card)
Payments by cash or credit card only. (All credit cards accepted)
Medical and vaccination records, if you have any.
You should have something to eat and drink before arriving for your examination, but please do not bring food into the office
A urine specimen is not needed.
 
 
 Travel Directions
 
By Subway  
1,2,3,9 trains to Chambers Street station. From Chambers St. and West Broadway, walk east on Chambers St. two blocks to Broadway. Turn left onto Broadway and walk uptown (against the traffic) three blocks. Our entrance is on the left in the block before Mc Donald’s at Thomas St.
A,C,E trains to Chambers Street station. Walk east 1 block on Chambers Street to Broadway. Turn left and walk uptown (against the traffic) 2 blocks on Broadway. Our entrance is on the left in the block before McDonald’s at Thomas St.
R W trains to City Hall station. Walk uptown on Broadway(against the traffic), cross Chambers St., and continue two more blocks. When you come to 26 Federal Plaza, cross to the other side of Broadway at Duane St. Our entrance is to your left in the block between Duane St. and Worth St.
4,5,6 trains to Brooklyn Bridge–City Hall station and J,M,Z trains to Chambers St. station. Walk west to Broadway. Turn right onto Broadway and walk two blocks to 26 Federal Plaza, cross to the other side of Broadway at Duane St. Our entrance is to your left in the block between Duane St. and Thomas St.
By Bus  
M1, M6 Downtown on Broadway. Get off at Worth Street. Walk towards Mc Donalds, we are in the middle of that block after passing Mc Donalsds.
M20 Downtown on 7th Avenue/VarickSt./West Broadway. Get off at Duane St. Walk east on Duane St. (uphill) two blocks to Broadway. Turn left onto Broadway and walk uptown(against the traffic)½ blocks. Our building is on your left between Duane St. ( before McDonald’s) and Thomas St.
M6 Uptown on Church Street. Get off at Worth Street. Walk east (uphill) 1 block on Worth to Broadway. Turn right onto Broadway. Our building is on the right in the middle of the block after passing Mc Denalals.Thomas.
